I believe the credential you seek is a "DDS" (Doctor of Dental Surgery) but not every DDS will administer deep sedation. I would suggest finding a dentist (with a very conservative approach) for an examination and cleaning, only. If that dentist recommends procedures which he or she can't perform, they will refer you to a doctor who can. I had a friend who was terrified of going to the dentist. She liked Dr. O'Leary in Lakewood.

http://www.ohdentalcare.com/ I was in a very similar situation as you, neglect ended up in a lot of cavities after high school. I'm 24 now and had to have 9 or 10 teeth drilled and filled about 3 months ago. This place was great, very friendly and they weren't judgemental. I also can't stand getting cavities drilled while I'm conscious and they will put you to sleep for the procedure, which they did for me. That being said, the sedation is fairly expensive but my teeth look 100% better and I'm not embarrassed about my smile any more.
