I'm sure there's lots of free ways to do this. But we paid https://www.uifcare.co.za/ to sort everything out for us - they include a legally acceptable contract and all the other details you need (regarding uif, they registered us for uif, etc as well). I considered it money well spent seeing as we had to balance out the amount of time it would've taken me to go the free version, and just following professionals in the field.
