We used Dr. Rohn from here: http://entkidsadults.com/ - Very, very happy with our experience. He and his staff are all so very nice. The only downside is we did experience a long wait one day, but otherwise, great. He even personally called us the evening after my toddler's tube surgery to check on him.

I know a good one in Fort Worth, but think he also has another location in the Metroplex. His name is John Fewins & he is excellent!!! He's my mother's ENT, but knows me well from me going with her to her apps. I highly recommend him!

Dr. Kirkham is ENT to the stars. Mick Jagger, Pattie LeBelle, Stevie Nicks (I think) and MANY more. He truly is one of the BEST in the world. Many singers who develop serious voice problems are patients of his. He keeps them making music:)
