Spence Richards, sports med doc at the Sports Medicine Specialty Group (offices in SLC and Bountiful). I've seen him for multiple injuries over the last 4 years - he'll absolutely work with you to keep you out there if at all possible. He's a triathlete, runner, etc so he understands.

I have had positive experiences with both Joel Nuttall at TOSH and Laura LaMarche at the U for physical therapy. But I would also try a chiro as well

Josh at RPT is great along with Josh at Hand and Ortho. Both places are AWESOME. Or Chris Harper at Alpine Physical Therapy in Sandy right next to Alta. He was my Physical Therapy teacher last year... he has TONS of experience and is the smartest guy I've ever met when it comes to PT!

Just got great help from the U orthopedic center for my shoulder http://healthcare.utah.edu/orthopaedics/ but I think you need a referral from your doctor to go there, YMMV. I was ignoring my pain until my better half made me go in and it is so much better now. Good for you doing something to improve your lot, I wish you good luck.

Most orthopedic doctors specialize. Do you know what kind he needs? If its knees, the best in Utah are Dr. Lonnie Paulos or Dr. Vernon Cooley. My sister has been to them both.
