Been going thru this with my Mom-the best therapy for her is going to be in a rehab place (like Evergreen) where she will stay and get intensive every day PT therapy. They have the equipment and resources to help people get better. Getting it at home isn't quite as effective. Good luck, its a never ending road.
Can anyone recommend a good sports physical therapist, just had rotator cuff surgery and hoping to find someone that's good and local.

Select PT, near Kohl's. I've used 3 different PT places for my 3 sports related injuries and they were by far my favorite.

Physical Therapy Partners on Elm Street in the St.Francis building. They are the best! My husband and I both had rotator cuff surgery in the last year. He went somewhere else and then switched over to PTP and was amazed at the difference in the quality of his therapy. Ask for Kim as your therapist.

I went to Select Physical Therapy on Elm street. They helped me recover from a neck injury after I was in a car accident. They were very good and very nice. If you make an appointment, ask for Heather. Shes amazing

All of the above are great. You don't need to spend a fortune on a ball. Also check Final Markdown in the mall.
